Also, when the Ambicor is inflated, one simply has a semirigid rod-type girth and the Ambicor does not increase further in girth nor any in length. When the Ambicor device is deflated, it's not quite as flaccid as the AMS-700 ultrex cylinders, for example. To combine the desired flaccid penis achievable with the inflatable devices while at the same time having a much more simple device, like the semirigid rods, the self-contained inflatable mechanical devices were developed, such as the currently used Ambicor or Duraphase devices. On the other hand, as Dr. Scott used to say, there is a "locker room" benefit, because with the inflatable cylinders in place, even when they are deflated, the flaccid penis has an increased degree of fullness and length. With the inflatable device, no one is able to tell that the patient has a prosthesis in place.
When the cylinders are deflated, the penis is flaccid, and when one activates the scrotal pump, fluid is transferred into the inflatable cylinders and the penis becomes erect. The inflatable device was intended to mimic much more closely the natural erection. My own personal experience with implants has been limited to the AMS products since I had the opportunity to train with Dr. Brantley Scott in the late 1970s and early 1980s. Mentor Corporation also makes an inflatable device that works well.
The AMS 700-Ultrex cylinders have been available since 1990 and expand in both girth and length, producing a more comfortable fit in the deflated stage. The 700-CX cylinders are heavier-duty cylinders that expand just in girth, or width, and is particularly helpful for diabetic patients with a lot of fibrosis of the wall of the corpus cavernosum, as well as for patients with Peyronie's disease. Over the past 15 years, the inflatable penile prosthesis, produced by AMS, has been improved and modified, and a lot of the early kinks in the hydraulic device worked out to the point where today it is a very reliable device. This device was manufactured by American Medical Systems, a company founded to support research and development of prosthetic devices for urology, including Dr. Scott's work at Baylor College of Medicine in Houston.
